Sport Physical
All students must have a sport's physical on file before being allowed to participate on any Wilson or Roosevelt Team. No student may try out unless a physical is on file. Tryouts for fall sports begin the first week of school. Physical forms are available in the Wilson or Roosevelt Offices. Physicals must be dated after April 15 for use in the next school year.
You may obtain the sport's physical from either your medical doctor or from the following:
